All Google Docs templates are fully editable and customizable. Some documents are in PDF which would require a PDF editor or copy/paste the information to a new document.

COST & HOW TO PURCHASE- $195

The cost is $195. I accept:

VERY IMPORTANT: You MUST include the email address in the comment section of the payment where you would like the Google Drive folder link sent. If you do not include the email address in the comment section of the payment and I have no way to contact you, I will be unable to complete your purchase.

Once the $195 payment is received by me, you will receive by email a Google Drive folder link within usually 5 hours if during business hours (M-F 9am-6pm) or by 24 hours if during evenings or weekends. Through this link you can easily view and download. From there you can edit most documents in Google Docs, Word, or other document editing program. Some of the documents are PDF. You can either copy the information to a new document or use a PDF editor.

A NOTE ON SHARING FORMS

The North American Registry of Midwives (NARM) does not prohibit sharing midwifery documents, as long as the midwife who wrote them gave permission for others to use them. If you are a Certified Professional Midwife (CPM) or a student in process of obtaining your certification with NARM, you should then have no issues if you choose to use parts of my forms and are audited.   If, however, you are a student and required to create your own documents as a school assignment, you are prohibited from submitting my work as your own.  Any student found plagiarizing will be reported to their school board and NARM. As my documents are well known in the midwifery community, plagiarizing would be a poor course of action for any student.
